Fordham von ReynFatuma's* doctors were completely perplexed. It was 2003 and she had returned to the DARDAR clinic in her hometown of Dar es Salaam, Tanzania three times that week with vague complaints of various pains and aches. Her doctors were considering whether these symptoms were (...) due to the initiation of her recent treatment with antiretroviral medications for her HIV infection or related to the last administration of the investigational tuberculosis (TB) vaccine that the DARDAR Health Study was evaluating. Fatuma's doctor, Dr. Lillian Mtei, consulted with Dartmouth's chief of Infectious Disease, Dr. Ford von Reyn, who happened to be in Dar that week on a site visit. Neither physician could determine the basis for her symptoms. Dr. Mtei returned to the exam room once more and this time emerged with the answer: Fatuma had just learned that her three year old daughter was HIV-infected, likely contracted from Fatuma during or shortly after she gave birth to her daughter. Fatuma believed that her HIV diagnosis was a death sentence for her daughter since antiretroviral therapy was not widely available for children.Dr. von Reyn, Dartmouth's lead clinical researcher in Africa and a father, knew too well the harsh reality of inequities in a resource-limited country. He turned to Dr. Mtei and said decidedly, "we need to do something about this."Thus the DARDAR Pediatric Program was born.Pediatric HIV: A Neglected DiseaseThe need for HIV treatment among children in Tanzania, as in most of sub-Saharan Africa, is compelling. At the end of 2009, approximately 160,000 children aged zero to fourteen in Tanzania were living with HIV (Joint United Nations Programme on HIV/AIDS [UNAIDS], 2011). Even with successful roll out of prevention of mother to child transmission programs in Tanzania, there were nearly 29,000 perinatal infections in 2009. As of December 2010, only 18% of these were enrolled in HIV treatment programs (UNAIDS, 2011; World Health Organization [WHO], 2011). Without antiretroviral therapy (ART), one-half of HIV-infected children will die before their second birthday. Since clinical research to determine efficacy and safety of new medical treatments is conducted almost exclusively in adults, provision of new medical treatments to [End Page 75] children always lags behind advances for adults. Furthermore, children are often underrepresented in programs for antiretroviral drugs for the following reasons: 1) they are not prioritized for care; 2) facilities lack infant diagnostic capabilities; 3) there is limited pediatric HIV training and consequently, a shortage of pediatric providers; and 4) pediatric antiretroviral formulations are frequently unavailable. The unacceptable delays in pediatric HIV drug development and treatment access led the international organization Drugs for Neglected Diseases initiative to publish an editorial in which they referred to pediatric HIV as a "neglected disease" (Lallemant, Chang, Cohen, & Pecoul, 2011).The Tip of the Iceberg: More than Just MedicinesWith a grant from the New York-based Foundation for the Treatment of Children with AIDS, the DARDAR Pediatric Program, a joint collaboration between Geisel School of Medicine at Dartmouth and Muhimbili University of Health and Allied Sciences, began seeing patients in May 2006. It was the first dedicated pediatric HIV Care and Treatment Center in Tanzania and operates in accordance with Tanzania's National AIDS Control Programme's guidance. Testing for both children and infants (the latter of which requires a more sophisticated assay) is available.The day the clinic opened we celebrated this collaborative effort that would provide HIV-infected children in Dar es Salaam access to antiretroviral treatment. One of our first patients was a six-year-old boy whose mother, like Fatuma, was a participant in our TB vaccine trial. She had been advised to bring her son to us for HIV screening. Her son looked healthy so she was surprised and understandably distraught when his test results indicated that not only was he HIV-infected, but his CD4 percentage was very low. Adequate interpretation of this topic requires understanding the traumas inflicted upon Africans sold into slavery, their existential predicaments before (...) and after emancipation, and the ongoing plight of their descendants whose psyches have been indelibly scarred by this traumatic history. Johnson avails himself of the resources of philosophical theology, psychoanalytic theory, and literary criticism to argue his thesis that African American religion expresses a tragic vision of reality. Besides providing an illuminating analysis of the traumatic experience underlying this religious vision, Johnson presents a compelling constructive proposal as to the broader implications of his thesis for Christian theology at large.As a student of David Tracy and Langdon Gilkey, Johnson sees the necessity for rigorous philosophical reflection in theology. Because the spirituality of the black church stands in some real tension with the traditional theological categories inherited from Europe, Johnson proceeds with “a phenomenological orientation grounded in a philosophical hermeneutical approach” that allows him to excavate the traumatic experiences informing this spirituality (4). He skillfully uses the concepts of mourning, loss, longing, and desire developed in psychoanalytical theory to probe the significance of trauma for grasping the dynamics of African American spirituality. He engages literary criticism and argues convincingly for a distinction between “tragedy” as a literary genre and the tragic vision it articulates. Indeed, the tragic vision is prior to tragedy. The tragic is not the encounter with meaninglessness per se but “the always already aesthetic representation of the encounter, which preserves its terrible truth in a transfigured vision of reality” (6). This vision affirms life while facilitating “the recognition and embrace of the darker, more fundamental truths of life” (7).Cognizant that other theologians have declared the incompatibility of Christian faith with a tragic vision of reality, Johnson believes otherwise. He insists that the tragic is fundamental to human experience as such, although it is frequently evaded by strategies of foreclosure—including those of Christian theology—that falsify the experience. Yet the distinctive thing about the black religious experience is the way African Americans made the Christian faith [End Page 215] their own by transforming it at a deep spiritual level through their tragic vision. Mildly critical of previous formulations of black theology, Johnson contends that this involved much more than the affirmation that God cares about the sociopolitical liberation of oppressed people. The Christian tragic vision of the black experience offers the possibility of a new basis for “theology in an African American key” (6). Since he understands himself primarily as a Christian theologian, however, Johnson addresses an audience consisting of more than fellow black theologians. Boston: Shambhala, 2006. 312 pp.It is important to make a number of things clear about the work under review before proceeding to a discussion of the parts of the book that bear directly on Buddhist-Christian relations. In the first place, the reader should know the identity of the author, Thinley Norbu. In order to (...) make absolutely clear who the author is, there are no less than four forewords to the book by His Holiness Penor Rinpoche (head of the Nyingmapa school of Tibetan Buddhism), Kathog Rigdzin Pema Wangchen Rinpoche (a prominent Nyingmapa lama), Alak Zenkar Rinpoche (another prominent Nyingmapa lama), and Tulku Thondup Rinpoche (a well-known Nyingmapa lama who has authored several valuable volumes on Vajrayāna Buddhism). Thinley Norbu is the son of the late head of the Nyingmapa school, H. H. Dudjom Rinpoche; like his father, Thinley Norbu is a very highly regarded lama with a wide following of disciples both in the Tibetan exile community and in the wider world community of Buddhist practitioners. The purpose of this book is to provide a working commentary on the Dudjom Tersar Ngondro, a well-known set of preliminary practices promulgated by Dudjom Lingpa (1835–1904), a “treasure revealer” (terton) in Eastern Tibet. This particular form of what are termed the “extraordinary preliminary practices” in the Vajrayāna has been transmitted to a large number of Nyingmapa school practitioners in recent decades. It has become so widespread that it is not unusual for photocopies of the prayers and practices to be given out to interested individuals without any particular initiation or other formality. I myself have [End Page 191] received the text a number of times, both as a formal practice from highly respected Tibetan teachers and in this very informal way from my personal friends among Nyingmapa monastics. Perhaps in response to the wide diffusion of this particular set of Vajrayāna practices, the author of this commentary wishes to clarify the unique features of the system revealed by the terton Dudjom Lingpa and widely taught by his own revered father, Dudjom Rinpoche (1904–1988). I would have to say that the parts of the book that comment on the practices (there is also a commentary on a related tantric visualization) are very well written and should be of great inspiration to those who have undertaken the arduous path of practice in this tradition.On the other hand, the author makes the commentary an occasion for discussing other concerns related to the relationship of Buddhism to other religions, to empirical science, and to the modern world. In these portions of the text, he continues in a polemical vein going back to his very troubling book Welcoming Flowers from Across the Cleansed Threshold of Hope: An Answer to the Pope’s Criticism of Buddhism, published in 1997.1 It is quite clear to careful readers of Pope John Paul II’s book Crossing the Threshold of Hope (New York, 1994) that there was no intention in that book for the Pope to give out a “final word” on any particular subject, much less to make infallible pronouncements on “faith and morals.” Rather, the book is to be taken as the sincere reflections of the Pope on a number of subjects that come up in both the Church and the world. As such, John Paul II’s book is a collection of essays reflecting a series of interviews with the Italian journalist Vittorio Messori, whose outspoken political and religious views are well known. The chapter on the Buddha caused a great deal of consternation in interreligious circles because the vocabulary employed seemed to imply a very negative assessment of Buddhism as a religion. However, it is also clear from certain comments in that chapter that the Pope is thinking critically about Buddhist teachings on salvation, a topic fundamental to any Christian theological engagement with other world religions.The fact that the terminology used by the Pope is imprecise unfortunately reflects the state of general knowledge about Buddhism in the secular and Christian world. Central to this end were Frege’s insights on quantiWcation, the notation that expressed it, the logicist project, and the extension of mathematical notions like function and argument to natural language. The long-awaited Cambridge Companion to Frege is a compendium of Fregean scholarship that rigorously explores these and similar topics; editors Thomas Ricketts and Michael Potter have compiled a comprehensive collection of fourteen essays that individually provide focused appraisals of a number of Frege’s most substantial insights. On the advice of this journal’s editor, I have limited my review to the connections (or rather dissimilarities ) that exist between Frege and Russell, relying exclusively on Peter Hylton ’s contribution to the collection, “Frege and Russell”. Following a brief consideration of the volume as a whole, I move immediately into a detailed explication of Hylton’s analysis. The volume includes among its goals the following two aims: (i) to provide, for advanced students and specialists, a conspectus of recent interpretation of Frege, and (ii) to dispel the intimidation felt by readers who confront a diUcult thinker.2 I single out these ambitions because they most clearly represent the volume’s strengths and shortcomings. Enthusiasts of Frege will not be disappointed by the multitude of rigorously and technically written essays, essays that contribute substantially and originally to the collective understanding of Frege and his work. Indeed, as one significant contributor has suggested, human rights have become, since the Second Vatican Council, the "central norms of (...) social morality." 1 In all of this concern for the place of rights within Catholic thought, however, very little work has been done to explore the pre-modern roots of this moral category. This essay attempts to address this issue. The essay has three parts. The first section considers characteristics of rights language within the thought of Thomas Aquinas. The second section looks at the use of rights within the intriguing controversy of the late thirteenth century surrounding apostolic poverty. Finally, developments in Catholic rights language found in the work of the Renaissance figures Vitoria and Suarez are studied. The essay narrates the development in the use of rights within the tradition while suggesting certain threads of consistency. Specific persons have specific rights in accord with their position within society. This section will examine the meaning of the term ius in Thomas. It will be shown that Thomas used ius primarily in an objective sense, that is, in terms of "that which is right," or " that which is fitting." It will also be suggested that Thomas used ius in something of a subjective sense as referring to personal moral claims. This latter use was, however, dependent on a prior understanding of "that which is right." The rise of “the rest,” especially China, has triggered an inevitable transformation of the so-called liberal international order. Rising powers have started to both challenge and push for the reform of existing multilateral institutions, such as the International Monetary Fund, and to create new ones, such as the Asian Infrastructure Investment Bank. The United States under the Trump administration, on the other hand, has retreated from the international institutions that the country once led or helped to create, including the Trans-Pacific (...) Partnership ; the Paris Agreement; the Iran nuclear deal; the Intermediate-Range Nuclear Forces Treaty; the United Nations Educational, Scientific and Cultural Organization ; and the United Nations Human Rights Council. The United States has also paralyzed the ability of the World Trade Organization to settle trade disputes by blocking the appointment of judges to its appellate body. Moreover, in May 2020, President Trump announced his decision to quit the Open Skies Treaty, an arms control regime designed to promote transparency among its members regarding military activities. During the past decade or so, both Russia and the United States have been dismantling multilateral arms control treaties one by one while engaging in new nuclear buildups at home. (shrink)

As part of the roundtable “International Institutions and Peaceful Change,” this essay examines the role of institutional soft balancing in bringing forth peaceful change in international relations. Soft balancing is understood as attempts at restraining a threatening power through institutional delegitimization, as opposed to hard balancing, which relies on arms buildup and formal alignments. We argue that soft balancing through international institutions can be an effective means to peaceful change, spanning minimalist goals, which aim at incremental change without the use (...) of military force and war, and maximalist goals, which seek more profound change and transformation in the form of continuous interstate cooperation aimed at a more peaceful and just world order. However, the success of soft-balancing strategies in fostering peaceful change varies widely, even in today's globalized and institutionalized international environment. We explore these variations and identify three conditions for success that can inform both academic analysis and political practice: inclusion, commitment, and status recognition. We draw lessons from two historical examples: the Concert of Europe in the early nineteenth century and the League of Nations in the early twentieth century, and discuss how current threats to the liberal international order challenge soft balancing for peaceful change. Physicist David Bohm and biochemist Ilya Prigogine began a dialogue that implied a deep, structuring, primordial harmony within life. In classical Chinese this harmony is referred to as Li, which also designates the elegant, natural pattern found in jade. This article emphasizes the ways that perception of primordial harmony gives way to a vision of possibility and to the creative intelligence and action necessary for meeting the challenges we face. Summary Transsexuals are individuals who identify as a member of the gender opposite to that which they are born. Many transsexuals report that they have always had a feeling of a mismatch between their inner gender-based ‘‘body image’’ and that of their body’s actual physical form. Often transsexuals undergo gender reassignment surgery to convert their bodies to the sex they feel they should have been born. The vivid sensation of still having a limb although it has been amputated, a phantom (...) limb, was first described by Weir Mitchell over a century ago. The same phenomenon is also occurs after amputation of the penis or a breast. Around 60% of men who have had to have their penis amputated for cancer will experience a phantom penis. It has recently been shown that a significant factor in these phantom sensations is ‘‘cross-activation’’ between the de-afferented cortex and surrounding areas. Despite this it also known that much of our body image is innately ‘‘hard-wired’’ into our brains; congenitally limbless patients can still experience phantom sensations. We hypothesise that, perhaps due to a dissociation during embryological development, the brains of transsexuals are ‘‘hard-wired’’ in manner, which is opposite to that of their biological sex. We go on to predict that male-to-female transsexuals will be much less likely to experience a phantom penis than a ‘‘normal’’ man who has had his penis amputated for another reason. The same will be true of female-to-male transsexuals who have had breast removal surgery. We also predict that some female-to-male transsexuals will have a phantom penis even although there is not one physically there. We believe that this is an easily testable hypothesis, which, if correct, would offer insights into both the basis of transsexuality and provide farther evidence that we have a gender specific body image, with a strong innate component that is ‘‘hard-wired’’ into our brains. This would furnish us with a better understanding the mechanism by which nature and nurture interact to link the brain-based internal body image with external sexual morphology.. (shrink)

Through a case study, this paper addresses ethical issues and dilemmas faced by a Family Therapist working in a Child and Adolescent Mental Health Service (CAMHS) in the National Health Service. When there are legal and societal obligations on parents/carers to ensure that the needs of children and young people are met within a family context, working with a young person in a health care setting oriented to the individual raises ethical dilemmas around consent. When the values of young people (...) and their parents conflict, legal, ethical and political issues can be raised. These have implications for the duties of health care professionals and the rights, interests and autonomy of the individual young people and their parents. The importance of justice to CAMHS practitioners' ethical decision-making about when to prioritize the individual over the family, or vice versa, is emphasized. (shrink)

One of the most important social cognitive skills in humans is the ability to “put oneself in someone else’s shoes,” that is, to take another person’s perspective. In socially situated communication, perspective taking enables the listener to arrive at a meaningful interpretation of what is said (sentence meaning) and what is meant (speaker’s meaning) by the speaker. To successfully decode the speaker’s meaning, the listener has to take into account which information he/she and the speaker share in their common ground (...) (CG). We here further investigated competing accounts about when and how CG information affects language comprehension by means of reaction time (RT) measures, accuracy data, event-related potentials (ERPs), and eye-tracking. Early integration accounts would predict that CG information is considered immediately and would hence not expect to find costs of CG integration. Late integration accounts would predict a rather late and effortful integration of CG information during the parsing process that might be reflected in integration or updating costs. Other accounts predict the simultaneous integration of privileged ground (PG) and CG perspectives. We used a computerized version of the referential communication game with object triplets of different sizes presented visually in CG or PG. In critical trials (i.e., conflict trials), CG information had to be integrated while privileged information had to be suppressed. Listeners mastered the integration of CG (response accuracy 99.8%). Yet, slower RTs, and enhanced late positivities in the ERPs showed that CG integration had its costs. Moreover, eye-tracking data indicated an early anticipation of referents in CG but an inability to suppress looks to the privileged competitor, resulting in later and longer looks to targets in those trials, in which CG information had to be considered. Our data therefore support accounts that foresee an early anticipation of referents to be in CG but a rather late and effortful integration if conflicting information has to be processed. We show that both perspectives, PG and CG, contribute to socially situated language processing and discuss the data with reference to theoretical accounts and recent findings on the use of CG information for reference resolution. This paper investigates whether an abstract linguistic construction shows the kind of prototype effects characteristic of non-linguistic categories, in both adults and young children. Adapting the prototype-plus-distortion methodology of Franks and Bransford (1971), we found that whereas adults were lured toward false-positive recognition of sentences with prototypical transitive semantics, young children showed no such effect. We examined two main implications of the results. First, it adds a novel data point to a growing body of research in cognitive linguistics and construction (...) grammar that shows abstract linguistic categories can behave in similar ways to non-linguistic categories, for example, by showing graded membership of a category. Thus, the findings lend psychological validity to the existing cross-linguistic evidence for prototypical transitive semantics. Second, we discuss a possible explanation for the fact that prototypical sentences were processed differently in adults and children, namely, that children’s transitive semantic network is not as interconnected or cognitively coherent as adults’. From February 2020, the COVID-19 pandemic led to closures of educational institutions to reduce the spread of infectious disease. This forced the U.S. education system into a massive experiment with online education. Despite conducting online bioethics education for nearly twenty years, our bioethics program, a joint endeavor of Clarkson University and Icahn School of Medicine at Mount Sinai, was not immune to this disruption because our curriculum features intensive, one-week onsite courses. Even in the face of historic disruptions, it is (...) vital to ensure minimal interruptions to teaching and assessing students to provide effective education. This paper reviews the steps we took to successfully convert the onsite components of our curriculum to a synchronous online format, and it focuses on how we preserved instruction and assessment of practical skills that comprise these courses’ core. It also explains how we fostered interactive classroom environments. (shrink)
